How to become a CCHCA Network Provider

Chinese Community Health Care Association offers a network of highly qualified and culturally sensitive providers to the Asian community. There are three categories of membership with CCHCA. Below are summarized requirements of these categories and how to go about becoming a Network Provider:

Associate Membership:

A physician may apply for Associate membership if he/she has completed at least one year as a member in good standing on Chinese Hospital's Medical Staff. For applications to join Chinese Hospital's medical staff, please contact Ms. Patricia Chung at (415) 677-2481 or via email to This e-mail address is being protected from spambots. Upon receiving Associate membership, physicians will receive a CCHCA Physician Handbook and Risk Management Manual, which contain important information regarding CCHCA's policies and procedures. Associate Members will be listed in our Physician Directory, which is distributed to the our patient members and available to the public. In addition, Associate Members will then be eligible to serve patients who have chosen CCHCA as their Medical Group. Payment for services performed (applicable only to authorized and covered services) are reimbursed at an enhanced rate. Details on additional benefits in becoming an Associate Member is available upon request.

Affiliate Membership:

An Affiliate Member is a physician (primary care or specialist) located in Northern San Mateo County who has privileges at an acute care hospital, but is not a part of Chinese Hospital's Medical Staff. Applicants  for Affiliate membership are encouraged to apply for Chinese Hospital's Medical Staff privileges, but it is not a requirement for eligibility.

The Affiliate membership designation is only for the Chinese Community Health Plan contract. Affiliate Physicians may not be assigned patients under any other CCHCA contracted health plans. This means Affiliate Physicians may only be listed in the CCHCA directory as a provider of services for patients who are covered under Chinese Community Health Plan.
