How to become a CCHCA Network Provider

Chinese Community Health Care Association offers a network of highly qualified and culturally sensitive providers to the Asian community. There are three categories of membership with CCHCA. Below are summarized requirements of these categories and how to go about becoming a Network Provider:

Associate Membership:

Physicians who receive associate membership will receive a CCHCA Physician Handbook and Risk Management Manual, which contain important information regarding CCHCA's policies and procedures. Associate Members will be listed in our Physician Directory, which is distributed to the our patient members and available to the public. In addition, Associate Members will then be eligible to serve patients who have chosen CCHCA as their Medical Group. Payment for services performed (applicable only to authorized and covered services) are reimbursed at an enhanced rate. Details on additional benefits in becoming an Associate Member is available upon request.

Affiliate Membership:

An Affiliate Member is a physician (primary care or specialist) located in Northern San Mateo County who has privileges at an acute care hospital.
