Become a Member of CCHCA
Simply choose Chinese Community Health Care Association as your group after enrolling in San Francisco Health Plan. If you are already enrolled and would like to switch to CCHCA, you may contact your health plan's customer service department (printed on your insurance card) to see if you are eligible to select CCHCA as your "Medical Group."*
