In partnership with our network of Providers, Chinese Community Health Care Association (CCHCA) provides quality medical care while striving to meet the needs of our community.  We are committed to promoting and strengthening the CCHCA Medical Group through managed care contracting opportunities with participating Providers.  Our Provider Relations team is dedicated to delivering excellent support, resources, and regular communication to our Providers to ensure a quality experience.

Q: Iā€™m interested in joining the CCHCA network. How do I become a contracted CCHCA provider?

A: Thank you for expressing interest in joining the CCHCA network. To apply for membership, please complete the CCHCA Physician Application Request Form and submit it along with the following supporting documents:

  • Letter of Intent

  • CV

Please note that completion of this form and its submission is not a guarantee of acceptance.   Your application request will be presented to the CCHCA Board of Trustees for review, and the Credentialing team will work with you on subsequent steps. Q: How do I sign up for the Provider Portal? What functions are available on the Provider Portal?

A: EZ-Net is the Provider Portal for CCHCA contracted providers. We highly recommend the usage of the provider portal to check the status of authorizations, claims, member eligibility, and to submit authorization requests. We also have a Provider Portal Demonstration Video for a quick refresher on different functions and features available on our portal. If you are interested in signing up for EZ-Net, please complete the Provider Portal Registration Form or contact Provider Relations at Provider.Relations@cchca.com or call 415-216-0088 ext. 107.

Q: Where can I locate the prior authorization form and the list of services/procedures that require prior authorizations?

Q: How do I file for provider disputes? How do I contact the Claims Department for claims status and status on disputes/appeals?
